Well-led
We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.
Our rating of well led stayed the same. We rated it as good because:
Leaders demonstrated a clear commitment to delivering high-quality care and driving continuous improvement. Governance and oversight processes were established across the service and supported the effective monitoring of quality, safety, patient experience and outcomes. Leaders had a good understanding of the service's strengths and the areas where further development was required.
We identified some areas where governance oversight and assurance could be strengthened, particularly in relation to the consistency of documentation, the accessibility of some clinical records and ensuring risk assessments were updated promptly following significant events. However, these issues were not widespread and did not compromise the delivery of safe care. Staff demonstrated a good understanding of patients' risks and care needs, and teams took appropriate action to manage identified concerns.
Our findings demonstrated that governance processes operated effectively at team level and that performance, quality and risk were managed well. Staff had access to the information they needed to provide safe and effective care and used this information to support clinical decision-making and service improvement.
Staff routinely collected and reviewed data relating to performance, safety and patient outcomes through regular audits. The service used this information to identify trends, monitor progress and drive quality improvement initiatives. Staff engaged positively with local and national quality improvement activities and demonstrated a commitment to improving patient care.
Managers worked collaboratively with other health and social care providers, including the local NHS mental health trust and community partners to support coordinated care and successful discharge planning. This partnership working helped to ensure patients received joined-up care and support throughout their treatment and recovery journey.
