Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. People and their relatives told us they had choice and control in relation to the care that was provided to them. People were actively involved in their initial care planning and reviews of care. A relative told us, “If there is a new plan I am the first to know, they ask me my opinions.”

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Everyone we spoke to told us that staff knew people well, and the provider was adaptable to people’s changing needs. A professional told us, “They [provider] go above and beyond, they work in line with people’s preferences.”

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. People were able to access their information in real time using an online system. Relatives could also access this where appropriate arrangements for consent were in place and managers had regular face to face and phone communication with the people they support.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. People told us that they were involved in all aspects of their care planning. The provider undertook surveys with staff and people to assess their effectiveness and to drive continued improvement.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Records showed that where necessary, the provider supported people to access services within the community where help was required with this.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. The provider demonstrated that they had undertaken tasks to ensure the people they supported had access to all resources necessary, for example the service ensured staff understood individual’s communicative needs so that people could always make their preferences known.

Planning for the future

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. The service was not supporting anyone with end of life care at the time of the inspection, however people had the opportunity to discuss their future plans and end of life care wishes if they wished to do so.
