Assessing needs

The service supported people to be involved with assessments and reviews of their needs and support. People had monthly reviews with their allocated keyworkers, which included reviewing care plans, goals, emotional wellbeing and activities, and were tailored to individual preferences and communication needs. Relatives told us, “[Staff] pick up on [person’s] needs and how they change” and “They regularly have reviews and involve us as far as possible”.

Supporting people to live healthier lives

The service empowered and supported people to manage their health and wellbeing needs.

Staff supported people to attend routine health appointments, for example, the dentist, opticians, and annual medicines reviews. When there were changes in people’s health, staff supported people to see relevant health professionals. When a person wished to access a health service independently, we saw staff had supported and empowered them to do this.

The Mental Capacity Act 2005 (MCA) provides a legal framework for making decisions on behalf of people who may lack capacity to do so. The MCA requires that, as far as possible, people make their own decisions and are helped to do so when needed. When they lack mental capacity to make a specific decision, any made on their behalf must be in their best interests and as least restrictive as possible. We found processes and records relating to capacity assessments, best interest decision making and consent were inconsistent. This meant we could not be assured decisions had always been made in line with the MCA.

However, staff we spoke with shared examples of how they supported people to make their own decisions and demonstrated a clear understanding of consent and upholding people’s right to refuse. This was reflected in feedback from people and relatives, for example, a person told us, “I am able to make my own choices”. We observed staff providing choice and asking for consent before supporting people. The registered manager was responsive to feedback about processes and told us what action they were taking to improve.
