Equity in experiences and outcomes
Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
The provider gathered feedback from people with surveys which were accessible and written in an easy-to-read format, to ensure everyone had an equal opportunity to contribute.
Records were person-centred and people were given choices in how they wanted their care delivered to achieve the best outcomes for them. One staff member told us how a person had requested a change to support their health and well-being. In addition, 1 person was being supported to maximise their wellbeing by doing exercises. A staff member told us, “[Name} is enjoying the weather outside and we helped her with her exercises.”
People were appropriately referred to health professionals, such as the district nurse and social workers. Advice was followed to support good outcomes for people and this was recorded.
