Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.People had individualised care plans reflecting their personal history, preferences, and specific needs. Care plans were reviewed regularly, and each person was assigned a key worker to help ensure they were actively involved in planning their care and support. This helped ensure care records were kept up to date. People were encouraged to personalise areas of the service. For example, one person who attended a local gardening group looked after plants in several spaces around the service. Bedrooms were also personalised, with items like photos, artwork, and other belongings important to each person.
