Learning culture
The provider had systems to learn when things went wrong. There was a procedure to investigate accidents, incidents and complaints. The manager discussed lessons learnt with staff and together they reflected on best practice. Relatives told us they were able to discuss when changes or improvements were needed.
Safe systems, pathways and transitions
The provider had systems to support safe transitions and pathways. They carried out assessments of people’s needs. The manager liaised with them, their families and representatives to plan transitions to the service. The manager recorded people’s needs, including how they communicated. This information was available for other professionals to view to help them understand people’s needs.
Safeguarding
The provider had systems to help safeguard people from the risk of abuse. Staff undertook training to understand about these procedures. Staff were able to explain how to recognise and report abuse. The provider had information about the local safeguarding authorities and how to contact them if needed. Relatives told us people were safely cared for. They explained they had information to help them understand about abuse so they could recognise this.
Involving people to manage risks
The provider assessed and managed risks to people’s safety and wellbeing. The manager met with people and their relatives to discuss how they wanted to be cared for and any risks involved with this. They helped people be independent and take risks when appropriate and this is what they wanted. Risk assessments and plans were regularly reviewed and updated. Staff worked with relatives to help identify and manage new risks. Relatives told us they thought risks were well managed. Their comments included, “Staff give me peace of mind, and I feel I can trust them” and “[Care worker] keeps [person] safe at all times.”
Safe environments
The provider assessed people’s home environments to make sure they were safe and risks were identified. They worked with people, their relatives and, when appropriate other professionals, to ensure people had the right equipment to meet their needs.
Safe and effective staffing
There were systems to check staff suitability when the provider recruited them. However, these had not always been followed. Staff had not always given clear information on application forms. Additionally, the provider had not always verified references. Failure to carry out robust checks meant there was an increased risk staff were not suitable. We discussed this with the manager. They carried out other checks before recruitment and a range of checks during staff inductions. This had helped to mitigate risks. The provider agreed to review the practices during staff recruitment to ensure further checks were made.
There were enough staff to care for people. Relatives told us people were cared for by the same regular care workers. They arrived on time and did not rush people. Comments from relatives included, “[Care workers] do extra tasks and adapt care to what we need”, “We have the same regular care workers”, “They are always on time” and “[Care worker] is very reliable and thorough.”
Staff undertook a range of training relevant to their role. The manager supervised them and worked with them to improve their skills and knowledge. Staff told us they felt supported and had the training they needed. A staff member commented, “I always get learning opportunities from management. I get feedback and suggestions about how I can improve my work. I have supervision meetings and informal discussions with managers.”
Infection prevention and control
There were systems to help prevent and control infection. Staff undertook training to help them understand these procedures. Staff were equipped with personal protective equipment (PPE) to use when needed. Managers carried out checks on staff to make sure they were following procedures. Relatives told us staff kept people’s home clean and followed good hygiene practices.
Medicines optimisation
People received their medicines safely and as prescribed. The manager worked with people and their relatives to assess risks relating to medicines and to understand their medicines needs. They recorded clear information in care plans. Staff completed training to understand about safe medicines management. Staff completed records to show when they administered medicines. The manager carried out audits to make sure medicines were safely managed.
