Person-centred Care

Staff provided person-centred care which met people’s needs and reflected their choices. People were happy with the care they received. They told us the provider was flexible when they requested changes. Comments from people and their relatives included, “[The provider] is open to adjusting things if we need” and “When we wanted to change times, we just spoke with [the manager]. They were very accommodating.”

Care provision, Integration and continuity

People received continuity of care. They were supported by the same regular care workers. Staff worked in collaboration with external health and social care professionals. They shared information to ensure people received continuity of care.

Providing Information

People had the information they needed about the service. The provider supplied people with a file of information, including key policies and a copy of their care plan. People told us they were able to request additional information if they needed.

Listening to and involving people

The provider listened to people and involved them in planning their care. People and their relatives told us managers were available when they needed them. They explained they regularly visited and telephoned them to ask for feedback. A relative commented, “[Manager] is easy to contact and will always pick up on anything.” There was a suitable complaints procedure. People had a copy of this. People and their relatives knew how to make a complaint. They told us they had not needed to.

Equity in access

The provider ensured equity in access. Staff supported people to access the services they needed. For example, attending medical appointments and being out and about in the community when this was part of their planned care.

Equity in experiences and outcomes

The provider ensured equity in experiences and outcomes. The manager assessed and planned for people’s diverse needs. They made sure barriers to providing care were identified and mitigated. Staff liaised with people and their families to make sure they felt their needs were being met. Staff undertook training to understand about equality and diversity.

Planning for the future

The provider helped people plan for the future. They asked them about their wishes for future care and objectives they wanted to achieve. Staff provided care which helped people work towards these objectives. At the time of our assessment, no one using the service was being cared for at the end of their lives. Staff had undertaken training about providing end of life care.
