Our view of the service

Tyne Grange provides accommodation with personal care for up to 21 people. 18 people were living at the home when we visited. The service provides nursing and personal care for people with mental health needs. We visited the home on 6 February 2026. This unannounced assessment was triggered to review the ratings for 3 key questions, to check whether the home was safe, effective and responsive. We reviewed the key questions relating to caring and well-led during a recent focused inspection.

The service was outstanding in delivering responsive care. Staff understood each person well and ensured care was shaped around what mattered to them. People experienced bespoke approaches, including personalised communication support, carefully matched keyworkers and goal focused recovery plans. Information was accessible and tailored, enabling people to be fully involved in their care. Leaders fostered a culture of listening and co production; feedback from people directly informed improvements to catering, activities, staffing and communication systems.

The provider ensured people’s needs were assessed and reviewed regularly, with multi disciplinary professionals contributing to care planning. Staff worked effectively across teams and services to ensure people received consistent care. Care was personalised to people’s health, wellbeing and communication needs. People were supported to live healthier lives and achieve exceptionally positive outcomes. Outcome were monitored and adjusted to reflect changes in people’s needs.

People had timely access to the support they needed, and staff worked proactively to identify and address inequalities in people’s experiences. Staff adapted care to remove barriers linked to language, mobility, cultural needs or digital confidence. People consistently told us they felt listened to, understood and empowered
 

People's experience of this service

People consistently described an exceptional experience of living at Tyne Grange. They told us staff knew them well as individuals and treated them with dignity, kindness and respect. A person said, “Staff treat me kindly and I have good relationships with staff who have been here a while.” People felt safe, valued and confident staff would listen and act if they had concerns. They also said the home was always clean and well maintained, which helped them feel comfortable and secure.

People said their daily lives were meaningful, enjoyable and shaped around their choices. They benefited from highly personalised communication support which helped them understand decisions and feel in control. A person said, “I feel comfortable talking to staff because they know me well and I feel safe.” Management had made improvements to catering, including redesigned menus and culturally familiar meals, directly in response to what people said and this had improved their dining experience.

People consistently described staff as available, supportive and focused on helping them achieve their goals. People felt actively involved in shaping the service through the service user council and ambassador programme. They told us they felt listened to, empowered and positive about their outcomes, with many saying there was nothing they would change about living at Tyne Grange.
