Assessing needs
The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.
Delivering evidence-based care and treatment
The provider planned and delivered people’s care and treatment with them, including what was important to them. Systems were in place to ensure staff were up to date with evidence-based guidance and legislation.
However, the provider did not always follow national evidence-based guidelines in the provision of some care and treatment such as infection prevention and control and medicines management national guidance.
How staff, teams and services work together
The provider had processes in place, so information could be shared between the service and the patient’s GP.
Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control.
Monitoring and improving outcomes
The service monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.
Audits, such as post-operative complications, had been completed and identified improvements. The findings had been followed up to check improvements had been actioned.Consent to care and treatment
The service told people about their rights around consent and respected these when delivering person-centred care and treatment.
Staff understood and applied legislation relating to consent. Capacity and consent were clearly recorded, and staff had completed training to support this.
