Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. The provider supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Nursing staff conducted carer reviews annually, and information for carers was available on the practice website. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. Information about these initiatives was available on the practice website. The practice had developed resources which informed patients of local groups they could join, such as walking groups, to promote independence and healthy living.
Monitoring and improving outcomes
The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The provider met national targets for the uptake of childhood immunisations. The provider performed better than the England average in relation to screening for cervical cancer. Practice leaders had introduced a new protocol to support people who did not attend cervical screening appointments. This included arrangements to support people experiencing menopause to attend comfortably.
