Learning culture

Score: 3

The provider had systems to learn when things went wrong. They investigated all incidents and whether anything could be done differently. They kept records of these to help identify any trends. Managers supported staff to work in a consistent way. Relatives and people using the service told us the staff discussed safety with them and made sure their views were considered when planning care or responding to incidents.

Safe systems, pathways and transitions

The provider supported people to have safe transitions between services. People and their relatives told us the registered manager met with them to carry out an assessment of their needs. They said staff supported them to liaise with healthcare professionals when needed.

Safeguarding

The provider had systems to help safeguard people from the risk of abuse. Staff undertook training about these. The provider shared information about how to recognise and report abuse with staff, people using the service and relatives. People and their relatives told us they were safely cared for. A relative commented, “Person is definitely safe with the staff.” Staff were able to describe what to do if they suspected someone was being abused.

Involving people to manage risks

The provider helped people to identify risks to their safety and wellbeing. Managers assessed these and created plans to help mitigate risk. People using the service and their relatives told us risks were well managed. A person explained how staff supported them to manage their anxiety and feel safe. Comments from relatives included, “[Person] is at risk of falls. The carers are very good and help keep [them] safe” and “Staff support [person] to move around safely so they do not fall.”

Safe environments

The provider assessed people’s home environments to help make sure these were safe. When they identified risks, they spoke with the person and/or their relatives about these. They had recommended people contact external companies, when needed, to help resolve risks within their homes or with equipment needs.

Safe and effective staffing

Score: 3

The provider deployed enough staff to meet people’s needs. People told us they were cared for by the same group of familiar care workers. They explained staff arrived on time and did not rush. Comments from people included, “They organise care in such a way that there is always cover”, “I always know the staff. There are the same regular carers” and “They are flexible and will change shifts according to my needs.”

The provider had recruitment systems to ensure staff were suitable. These included a range of checks. New staff completed an induction and undertook training to help them understand their roles and responsibilities. Staff explained they had regular support and supervision. The registered manager carried out checks on staff at work to make sure they were following best practice and providing good care.

Infection prevention and control

There were procedures designed to help prevent and control infections. Staff undertook training about these. The provider ensured staff had access to personal protective equipment (PPE) when needed. People told staff washed their hands and wore PPE such as gloves.

Medicines optimisation

At the time of our assessment, staff were not supporting anyone to take their medicines. There were procedures for managing medicines in a safe way. Staff had undertaken training to understand about safe medicines management.
