The provider did not always have clear responsibilities, roles, systems of accountability or good governance. They did not always act on the best information about risk, performance and outcomes, or share this securely with others when appropriate.

The providers governance systems had failed to ensure systems and equipment in place to detect and prevent the spread of fire in the home were adequately maintained and remained fit for purpose. In addition, they failed to ensure when changes were made to the infrastructure of the home, these were done in such a way that didn’t impact on fire safety. For example, the provider had recently installed solar panels on the roof but had failed to ensure the fire-resistant walls in the roof space were not compromised during the installation. The registered manager told us they had improved auditing processes.
