Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. This is the first assessment for this service since their registration in November 2019. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty.

Systems were in place that enabled people and staff to raise and report safety concerns and incidents. Although none had been reported in the past 12 months, the provider’s policy outlined how these would be investigated, and how any learning identified would be used to support continuous improvement in staff practice and ensure people’s safety. A senior staff member told us, “Training is a key area here for us to help staff understand how they must report things to us.”

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

The provider obtained information about people’s care and support needs and risks to their safety, before they started to use the service. This information was used to develop care and risk management plans for people which were shared with staff before they commenced a package of care. Staff worked with the relevant healthcare professionals, when required, to make sure care plans and risk assessments remained up to date with information about people’s current needs and risks to their safety.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect.

People had no concerns about the staff providing their care and support. A person told us, “I feel very safe with them and very comfortable with them.”

Staff received training to safeguard people from abuse. Staff were confident raising and reporting concerns when needed. A staff member said, “I follow safeguarding policies and procedures at all times. I remain vigilant for any signs of abuse or neglect and report concerns immediately to my manager or safeguarding lead.”

There had been no safeguarding concerns raised about people in the last 12 months. Senior staff understood their responsibility to make timely referrals when needed and to work proactively with the relevant agencies to ensure people were safeguarded from further risk.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

People were supported to stay safe whilst doing the things that mattered to them. A person told us, “If I want to go out and buy things they help me to do this and help me use my bank card. I am not worried at all.”

Staff understood how to manage identified risks to people. A staff member told us, “I review care plans and risk assessments before providing care and stay alert to any changes in a person’s condition. I also communicate with colleagues and managers to ensure I am aware of any updated risks.”

Senior staff assessed, monitored and reviewed risks to people at appropriate intervals.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

The provider assessed potential risks from people’s living environment and put measures in place to reduce these and keep people safe. They reviewed these arrangements at regular intervals to make sure the measures put in place to reduce risks, remained appropriate.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

People received care and support from a regular team of staff. Staff undertook relevant training to help them meet people’s needs.

Staff were supported to learn and improve in their roles through spot checks, supervision and appraisal of their work performance. A staff member told us, “I receive regular training to keep my skills up to date, along with supervision meetings where I can discuss my performance, concerns, and development.”

Checks were undertaken on staff that applied to work at the service, to make sure they were suitable to support people. These included criminal records and right to work checks. However, these had not always been effective as we found some discrepancies in the information about staff’s previous employment. The provider was able to provide explanations for this, and we were satisfied this had not put people at risk. They took immediate action after our site visit to strengthen their recruitment arrangements to ensure these would be more robust and effective going forwards.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

People’s living environment was kept clean and hygienic to prevent the spread of infection. A person told us, “They keep the house perfect and clean.”

Staff received relevant training and had access to resources, such as personal protective equipment (PPE) to help them reduce infection risks.

Senior staff undertook spot checks on staff to make sure their practice was in line with standards and national guidance for managing and minimising infection risks. A senior staff member said, “We do spot checks on staff and make sure staff understand how to prevent infection and are using PPE correctly. We look at their practice and make sure we see good practice. We have had no issues.”

Medicines optimisation

At the time of this assessment, people using the service did not require staff support to administer and manage their medicines. However, appropriate policies and procedures were in place to ensure the safe administration and management of medicines, should this need arise.
