Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question inadequate. At this assessment the rating has changed to requires improvement. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

The service was previously in breach of the legal regulation in relation to governance at the service. Although some improvements had been found, the service remained in breach of this regulation e improvements had been found, the service remained in breach of this regulation.

Shared direction and culture

Score: 3

The provider had a shared vision, strategy and culture and improvements had been made to embed this into the day to day running of the service, however this needed strengthening to ensure the management team were clear about their roles and responsibilities, and to ensure staff felt they were able to speak to any of the management team at any time.

The provider had acknowledged the challenges of the last year and beyond but remained committed to adopting a new, positive culture of which the people and staff living and working at Beechfields, could be proud of.

People told us they always felt they could speak up and that their voice would be heard.

However, we received mixed feedback from some staff who did not feel all the management team were approachable and would only go to certain leaders for support. We brought this to the provider’s attention who acknowledged the management team had worked hard on improving the service and this have been seen as a barrier whilst stricter processes were put in place. They acted proactively by increasing opportunities and further mechanisms for staff to speak up to further enhance the culture of openness they were trying to promote.

The provider valued diversity in their workforce. They worked towards an inclusive and fair culture by improving equality and equity for people who worked for them.

Although some staff did not always feel heard, we received positive feedback about staff being supported with protected characteristics and diverse needs. The provider had adopted new opportunities for staff to speak up and voice their opinions and concerns so any inequitable practices could be identified and resolved.

Governance, management and sustainability

The provider understood their duty to collaborate and work in partnership, so services worked seamlessly for people. They shared information and learning with partners and collaborated for improvement.

The provider had been working with the local authority in their quality improvement process to improve practices across the service and to provide good outcomes for people.

The provider was focused on continuous learning, innovation and improvement across the organisation and local system and had made some significant improvements since our last inspection. However, there were still elements of practice which needed improvement as highlighted throughout the report.

The provider was very receptive to our feedback about some of the issues we identified on inspection and took immediate actions to address these issues.
