Staffkeptup-to-datewith current evidence-based practice, andhad the skills, knowledgeandexperience to carry out their roles.

We looked at6patient care records. We found that patient clinical records were not in line with recognised guidance. The records reviewed did not consistently provide a clear contemporaneous account of the patient’s assessment, treatment and ongoing care. Improvements were also required to ensure the dentists justified, graded and reported on the radiographs they took.

Clinical records did not consistently demonstrate that valid patient consent had been obtained and documented before treatment was provided. Treatment plans and clinical notes did not always contain sufficient information to demonstrate the discussions held with patients, the treatment options considered, or the patient’s agreement to proceed.

Patients werereferred to primary and secondary carespecialistsfor treatment the practice did not provide. This included referring patients with suspected cancer under the nationalfaster diagnosispathways.

Improvements could be made to ensure a referral log is in place to support effective monitoring and tracking of patient referrals.

Staff felt the practice supported them to develop and enabled them to take on responsibilities.
