Staff and leaders did not consistently demonstrate active listening or responsiveness to the needs of people who may be more likely to experience inequality in care outcomes. As a result, care was not always tailored to reflect individual circumstances, and some people did not receive support that fully recognised or addressed their unique needs. Improvements were required to ensure that all people consistently received high-quality care from staff who understood their personal histories, preferences, and support requirements.

Care plans did not consistently include information about people’s cultural or religious beliefs, which limited the ability of staff to provide care that was respectful of individual identity and background. Although the provider had policies in place to guide equitable practice, including those on equality and diversity, equal opportunities, ethnic awareness, and anti-discrimination, these were not always reflected in day-to-day care planning. Staff had undertaken training to improve their understanding of equity and diversity, but further work was needed to embed this knowledge into practice and ensure that care delivery was inclusive and person-centred.

Whilst many people and relatives reported feeling safe, some expressed concerns about the impact of unfamiliar staff on the quality of care. In contrast, where people were supported by staff who knew them well, feedback was notably positive. One relative told us, “When he’s getting changed, they help him but prompt him at the right times so he can try himself.” This highlighted the importance of familiarity and personalised support in promoting independence and dignity.
