Independence, choice and control
The provider did not consistently promote people’s independence, which meant people did not always have full awareness of their rights or the opportunity to exercise choice and control over their care, treatment, and wellbeing. Care records lacked sufficient detail about people’s preferences and did not clearly outline how they wished to be supported or what tasks they could manage independently. This limited the ability of staff to tailor care in a way that empowered people and supported their autonomy.
Despite these gaps in documentation, we observed through daily care notes that staff made efforts to support people in maintaining control over their daily lives. One staff member described their approach, saying, “We have to try to understand her and give her time, we talked slowly to her, to help her feel safe and reassured.” This demonstrated a thoughtful and person-centred approach to communication, even though such needs were not always reflected in the formal care plans.
People and their relatives reported that staff respected their choices and supported decision-making. One relative told us, “They show genuine concern. They treat him gently. He has dementia so they have to ask things multiple times.” This feedback highlighted the compassionate nature of staff interactions, although the lack of detailed care planning limited consistency in promoting independence across the service.
