Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

Assessments of people’s care were undertaken when they first moved into The Hamiltons. We found these were detailed and took into account important aspects of people’s care, their life history and things they enjoyed doing. Monthly reviews of care plans were undertaken and any updates were documented.

Relatives told us they were involved with people’s care. One relative said to us, “The manager and staff have discussed (person’s) care with us and I can be guided by them. (Person) can be difficult and the staff appear to have the ability to calm (person) and help her understand.”

The provider did not always tell people about their rights around consent and did not always respect their rights when delivering care and treatment.

Mental Capacity Act (MCA) assessments were not always completed where restrictions were in place to demonstrate decisions were taken in people’s best interests. Sensor mats were used in people’s bedrooms to alert staff when people attempted to mobilise. The people living in these rooms lacked capacity to provide consent to these decisions. The registered manager gave us assurances that the relevant MCA assessments would be completed and that relatives would be consulted with as part of these decisions.

Deprivation of Liberty Safeguards (DoLS) applications were completed as needed and these were re-applied for when they had expired. Staff had completed DoLS and MCA training and understood about the legislation.
