Within 7 days of this condition taking effect the registered provider must ensure named persons are responsible for each area of practice management and clear lines of accountability are in place.
Within 7 days of this condition taking effect the registered provider must inform the Care Quality Commission of what actions they intend to take to implement a strengthened and sustainable operational and administrative leadership at Four Acre Health Centre. This is to ensure effective governance processes and improvement plans are implemented, which will address the failings and risks to patients in a timely manner. These actions should address the failings in:
a) Health and safety risk assessments relating to the premises and equipment.
b) The reporting and investigation of incidents.
c) Establish the missing information from staff recruitment files and perform adequate risk assessments in the absence of the required information.
d) Review the management of complaints and establish a process to manage, investigate and respond to all complaints that demonstrate learning for the practice.
e) Infection prevention and control risk assessments and oversight and monitoring of action plans.
The registered manager must within 28 days of this condition coming into effect review and update the system of handling significant events. This must include how all significant events are captured and recorded, how learning is implemented and shared, and how significant events are monitored to ensure required improvements are made and sustained. Monitoring must take place to ensure the updated system is effective.
