Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff and leaders promoted healthier lifestyles and preventative healthcare through a range of health promotion and screening initiatives. Patients were supported to access targeted health checks, including reviews for people with long-term conditions such as diabetes. The practice also identified and supported carers and provided information and resources to help people understand and manage their health needs, supporting informed decision-making and early intervention. The PPG had also supported health campaigns, including a medicines waste initiative. Performance demonstrated strong outcomes in preventative care, including meeting national targets in vaccination and immunisation indicators and strong results in blood pressure management and smoking-related measures. These arrangements supported people to access appropriate support, reduce health risks and improve long-term health outcomes.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The practice used clinical monitoring systems and population health data to identify need, monitor outcomes and support the management of long-term conditions. Structured recall systems were in place, including regular reviews for identified patients, supported by clinical monitoring and follow-ups to ensure patients received timely assessment and intervention. Performance data demonstrated positive outcomes, with the practice achieving above local and national averages.

The provider used clinical audits and quality improvement activity to monitor and improve performance. Performance data showed the practice was achieving positive outcomes in several preventative healthcare measures, including vaccination and immunisation programmes, blood pressure management and smoking-related indicators. Systems were also in place to monitor patients who did not attend appointments, including vulnerable patients and children, helping to reduce the risk of poorer health outcomes and support continuity of care.
