Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Risks to people’s health were part of the identified areas of people’s care needs that staff always monitored and shared with people. This also included those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity amongst other initiatives. We saw on the onsite visit a number of displays within the waiting area and in consultation rooms promoting people’s health.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met or exceeded national targets for the uptake of childhood immunisations. We found the service had in place a cancer screening improvement plan (2025/2027). As part of this plan, they had processes in place to follow up people on a monthly basis using a number of different methods. While the service performance for 2024/25 were slightly below expected target for bowel cancer screening, they had and met the target for breast cancer screening. Evidence was provided of the improvement they had made since putting their plan in place, for this current year. For example, bowel cancer screening was currently running at 71% for 2026 and breast cancer screening was at 72% for 2026, this was higher than the current England average for 2025. Cervical Cancer screening was also showing an improvement and if this continued the analysis for 2027 would demonstrate the service had met or exceeded the national target.
