Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control.
Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. We noted that the practice supported the Berkshire West Community Wellness Outreach Project, supporting residents most at risk of cardiovascular disease through community bases NHS health checks and holistic well being support.
Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. Smoking cessation support was available weekly within the practice, with 20 patients engaging in counselling during the last year. Drug and alcohol support was also provided fortnightly, with 16 patients accessing ongoing counselling during the past year. These initiatives helped improve access to preventive health care for groups who traditionally experience poorer health outcomes.
Health promotion was supported through the waiting room information display, patient leaflets, the practice website and social media.
We noted that the practice continually reviewed new opportunities to improve services and reduce inequalities for its patient population.
Monitoring and improving outcomes
The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The practice met national targets for the uptake of childhood immunisations and cervical screening.
The practice shared a range of clinical audits covering medicines management and specific health conditions, demonstrating their use of quality improvement activities to enhance outcomes for people using the service. GPs and the practice manager held regular clinical and multi-disciplinary meetings to review performance and monitor outcomes.
The practice contributed to the West Berkshire learning disability and severe mental illness workstream, which aimed to increase annual heath check uptake and improving accessibility
