During an assessment of Community-based substance misuse services

Date of assessment 18 to 27 February 2026.

We rated the service as good. The service was previously rated good. Staff treated all patients with compassion and kindness. People were supported to have choice and control, were involved in planning their care and staff were fully invested in clients’ wellbeing. The service identified and supported the wider health needs of clients, wellbeing and communication needs. The service worked in collaboration with external partners, and the organisation encouraged improvement initiatives.

Mental Health Act and Mental Capacity Act Compliance Summary

The staff we spoke with had a good understanding of mental capacity and were confident about how to assess it. Staff told us they took all practical steps to enable people to make their own decisions and provided good examples regarding raising safeguarding concerns.

People using the service had easy access to information about independent mental health advocacy.

We reviewed 5 care records and found evidence that people's needs were met in line with current guidance and by undertaking capacity assessments and best interest decisions when appropriate.

Multi-disciplinary team discussions involved the client and their family, where appropriate, in decision making and planning care and treatment.

We assessed We are With You North Lincolnshire from 18 to 27 February 2026.

The service was previously rated Good in the last report published on 17 March 2020.

We are With You North Lincolnshire was registered by CQC on 10 September 2018 and is a community substance misuse service located in Scunthorpe. It is provided by the national drug, alcohol and mental health charity We Are With You. The service is commissioned by the local authority to provide community services for adults experiencing problems with substance and alcohol use. The service delivers both pharmacological and psycho social interventions to address harm reduction through recovery and rehabilitation. At the time of our assessment, they were working with approximately 700 clients.

The service is registered to carry out the following regulated activities:

At the time of the inspection, the service had a registered manager.

At this assessment we assessed 1 assessment service group: Community based substance misuse services.

We rated the service as Good.

During a routine inspection

We rated Addaction North Lincolnshire as good because:

  • The service provided safe care. The premises where clients were seen were safe and clean. The number of clients on the caseload of the team, and of the individual members of staff, was not too high to prevent staff from giving each client the time they needed. Staff assessed and managed risk well. They followed good practice with respect to medicines management, safeguarding and appropriately investigated incidents to ensure lessons were learnt and shared.
  • Staff provided a range of treatments suitable to the needs of the clients and in line with national guidance about best practice. They engaged in audits and benchmarking to evaluate the quality of the care they provided.
  • The team included all specialists required to meet the needs of clients under their care. Managers ensured these staff received training, supervision and appraisal. Staff worked well together as a multidisciplinary team and with relevant services outside the organisation.
  • Staff treated clients with compassion and kindness. They understood the individual needs of the clients and actively involved them in their care.
  • The service was easy for clients to enter into treatment. Staff planned and managed transfers and discharges well and encouraged clients to engage with the wider community. They met the needs of clients with complex needs and in vulnerable circumstances who often found engagement with services difficult. They listended to, investigated and learnt from concerns and complaints.
  • The service was well led and the governance processes ensured that its procedures ran smoothly.

However:

  • Recovery plans and case notes did not reflective of the holistic conversations which had taken place. They did not contain goals which were specific, measurable or timely.
  • Client’s privacy could be compromised due to rooms not being soundproof.
