Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking, tackling obesity and other initiatives. We observed several health and wellbeing information displays in the reception area designed to support patients. Patients we spoke with were positive about the service and the support available.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service met national targets for the uptake of childhood immunisations. The service performed better than the England average in relation to several cancer screening programmes, including screening for breast and bowel cancer. For example, the practice achieved 80.6% for breast cancer which was higher than the national average of 70.4% and the practice’s achievement for bowel cancer was 77.4% which was higher than the national average of 71.8%. We also found from data produced by NHS England on cervical screening that this practice performance was above the expected targets. The service demonstrated a good understanding of the barriers affecting screening uptake and had implemented targeted actions to encourage attendance. Performance data indicated that these actions had been effective in improving participation rates. The service worked closely with other agencies such as the Primary Care Network (PCN) to support patient care.
