Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
We found that supporting people to live healthier lives was an important part of the service provided. We saw displays in the waiting area aimed at providing information on healthy living. The provider had a section for men and a section for women. The service promoted healthier living national priorities like stopping smoking initiatives to help improve how their patients lived. Staff we spoke with gave examples of how they supported people.
Monitoring and improving outcomes
The service did not always routinely monitor people’s care and treatment to continuously improve it. They did not always ensure outcomes were positive, consistent or the expectations of people themselves.
While the service did not quite meet the national target for the uptake of childhood immunisations for the period April 2024 to March 2025. The provider had managed to improve patient attendance in other areas, which was an achievement. The service showed the actions they were taking to improve their targets. For example, speaking with parents face to face when they were in the service for other care and treatment, carrying out regular monthly recalls, attending community events which gave them the opportunity to share the importance of immunisations where parents were also in attendance, and working closely with the Integrated Care Board (ICB) to look at ways they may not have considered to reach out to parents. When parents had made it clear they were not going to attend these appointments, the provider had in place a disclaimer which parents would sign. We also found breast screening and bowel cancer screening data for the period 2024/25 were below the expected target. For breast screening the England national target was 70.4% and the provider was 60% and for bowel cancer screening the England national target was 71.8% and the provider was 61%. Cervical cancer screening data carried out in June 2024 was also below the national target. The England expected target was 80% for women aged 25 to 49 and for women aged 50 to 64. The provider was 58% for women agreed 25 to 49 and for women aged 50 to 64 was 72.1%. The provider was already aware of this and provided their action plan for improving patient attendance for these appointments. They showed us an improvement for September 2026, but this was still below the national target for women aged 25 to 49.
