The service had a proactive and positive culture of safety, based on openness and honesty. They listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

Leaders encouraged staff to raise concerns when things went wrong. The provider had systems for staff to report incidents, near misses and safety events. The practice had recorded 64 significant events in the last 12 months that demonstrated appropriate action had been taken. People felt supported to raise concerns and felt staff treated them with compassion and understanding. Staff felt there was an open culture, and that safety was a priority. Analysis was undertaken and learning from incidents and complaints resulted in changes that improved care for others. When things went wrong, staff apologised and gave people support.

The provider detected and controlled potential risks in the environment; however, they did not always make sure equipment, and facilities supported the delivery of safe care. For example, clinical waste bins were not locked, the ramp to the front of the building was not fully accessible, access to the waiting room was not fully automated and there was no bell to request assistance with entry for patients with restricted mobility. These observations were discussed during our assessment. The provider was receptive to our feedback, and we received appropriate assurance the issues would be addressed. Following our assessment, we received confirmation that several issues had been addressed and that appropriate timescales were in place to ensure the remaining actions were completed.

Regular checks were conducted on the premises, facilities, and equipment. Contracts were in place to ensure the premises were clean and well maintained.Health and safety assessments and procedures were in place. Staff had been provided with health and safety related training including fire safety, infection control, and manual handling.There was a business continuity plan in place to provide guidance for dealing with a major disruption to the service, for example an IT failure.

The service made sure there were enough qualified, skilled, and experienced staff, who received effective support, supervision, and development. They worked together well to provide safe care that met people’s individual needs.

There were a range of clinical and non-clinical roles within the practice. We found training was up to date, learning needs and development of staff was managed appropriately, and staff were working within their agreed areas of competence.

A recruitment policy was in place, and a review of personnel files confirmed safe recruitment was consistently followed. New staff participated in a structured induction and were required to complete mandatory training that was regularly refreshed.
