Governance, management and sustainability

The service did not always have clear responsibilities, roles, systems of accountability or good governance. They did not always act on the best information about risk, performance and outcomes, or share this securely with others when appropriate.

The provider had made some changes to the governance processes since our assessment of December 2025. However, there was work to be done in this area. The policies and procedures they had in place were not always being followed correctly.

There was a recruitment policy in place but this was not being followed correctly. New staff have been recruited by the provider since the last assessment. Not all background and reference checks had been completed as per the providers own recruitment policy before the new staff were in post. The Infection Prevention Control (IPC) policy that was in place was not being followed correctly. The lack of up-to-date IPC audits and accurate hand-washing audits meant that the provider did not have a clear understanding of risks posed by staff not following the national guidelines on IPC.

Disclosure and Barring Service (DBS) checks were completed for all but 1 staff member who had been employed by the provider since November 2014.

Not all staff had received an appraisal in the last 12 months, with 1 staff member not having one since they started employment in November 2014.
