Staff supported people to live healthier lives, monitoring their care and treatment to ensure they received positive and consistent outcomes. The provider undertook audit activity to make improvements in care, demonstrating a commitment to ongoing learning and service improvement.

Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.
The practice waiting area and the practice website provided a wide range of health information for patients.
We saw evidence to confirm people benefited from coordinated, proactive care focused on prevention, accessibility, and improved health outcomes.
A Social Prescriber worked at the practice (Social prescribers can help people by finding appropriate support or external organisations to compliment other care arrangements). This meant staff were able to refer people with social needs, such as those experiencing social isolation or housing difficulties.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The provider had a strong focus on prevention and early intervention and there was a high uptake of screening for specific conditions and childhood immunisations. As part of our assessment, we reviewed the practice performance data for childhood immunisations. The World Health Organisation (WHO) recommends a rate of 95% for all routine childhood vaccinations. The practice had met 95% of the target for 4 out 5 routine vaccination indicators.
