Supporting people to live healthier lives

The practice supported people to manage their health and wellbeing to maximise their independence, choice and control. The practice also supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The practice offered patients a range of health assessments and reviews which included NHS health checks, and learning disability health checks. Staff had been trained in care navigation skills, and were therefore better able to signpost patients to more appropriate providers of care such as pharmacies for identified conditions. The practice had developed a bespoke online care navigation platform to support effective triage and the signposting of patients. We were informed that the practice had organised and hosted a dementia awareness day for carers and patients in 2025. The practice website and onsite noticeboards carried information to promote healthier lifestyles.

Monitoring and improving outcomes

The practice routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

We saw that there were effective processes in place to support patients with specific needs, including those with long-term conditions. The practice either met or exceeded national targets for the uptake of childhood immunisations. We saw that 4 of 5 measures exceeded the World Health Organisation (WHO) recommended rate of 95% for all routine childhood vaccinations, and 1 measure met the 90% minimum target. In addition, the practice performed better than the England average in relation to patient participation in national cancer screening programmes, including screening for breast, bowel, and cervical cancer. For example, 82% of eligible women aged 25 to 49 years, and 86% of those aged 50 to 60 years had undertaken cervical screening, which was above the national target of 80%. Results from the 2025 National GP Patient Survey showed that patients felt that the practice delivered care and treatment which met their needs. We saw that 92% of respondents felt that their needs were met during their last appointment, compared to local and national averages of 90%. Data provided to us by the practice showed that in the last 12 months they had completed 95% of their annual reviews for eligible patients with a learning disability. Recall processes were in place to ensure that patients received necessary monitoring in line with national guidance. The practice used a care coordinator to assist vulnerable patients or those who required additional support.
