Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and, where possible, reduce their future needs for care and support.

Staff were proactive in identifying risks to people’s health and wellbeing, including those with caring responsibilities and people approaching the end of their lives. The nursing team promoted healthy lifestyles through routine contacts and annual reviews, providing support with smoking cessation, weight management and participation in national screening programmes. Care coordinators actively encouraged uptake of cervical, bowel and lung cancer screening, particularly among patients who may face barriers to accessing care.

The provider supported national health improvement initiatives aimed at improving population health and wellbeing, including tackling obesity and promoting physical activity. Staff coordinated a regular community running initiative in the local park, which encouraged patients to become more active and support their wellbeing.

People could also access social prescribing services through the Primary Care Network (PCN) to support their wider health and wellbeing needs. These services provided tailored support and signposted people to relevant community and voluntary sector resources to help address non-clinical factors affecting their health and wellbeing. Health promotion campaigns and digital support workshops further helped people access information, services and support to manage their health.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to improve outcomes and ensure care remained effective and responsive to people’s needs.

 

The provider monitored performance against national screening and immunisation programmes and generally performed well against national indicators. Available data showed that childhood immunisation uptake was high, with 100% of children aged 5 having received 2 doses of the measles, mumps and rubella (MMR) vaccine, exceeding the World Health Organization target of 95%.

 

The provider also monitored cervical screening uptake.According to NHS England data (2024), 79.0% of eligible women aged 25 to 49 had received adequate cervical screening within the previous 3.5 years, which was slightly below the national target of 80%. Uptake among women aged 50 to 64 was 82.7%, exceeding the 80% target. More recent data supplied by the provider that is yet to be verified or published demonstrated that cervical screening uptake among women aged 25 to 49 now exceeded the national target. Uptake among women aged 50 to 64 also exceeded the national target.

 

Overall, the provider demonstrated effective monitoring of national screening and immunisation programmes and achieved outcomes that met or exceeded relevant national targets.
