Freedom to speak up
The service fostered a positive culture where staff felt they could speak up and be heard.
Freedom to Speak Up arrangements were in place both internally and with the Integrated Care Board (ICB). Staff were aware of these processes, felt confident to raise concerns or suggestions, and reported that regular updates and meetings kept them informed of outcomes.
Workforce equality, diversity and inclusion

Governance, management and sustainability
The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.
A clear leadership structure was in place. Following the previous assessment, leaders implemented improvements, including the recruitment of 2 clinical leads, to support staff, and strengthen continuity and visible clinical leadership. Staff reported positive experiences of leadership and welcomed the changes and support provided by the clinical leads.
Regular practice meetings were held to discuss clinical concerns and emerging risks. Actions from these meetings were clearly recorded and communicated to staff.
