We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

Monitoring and improving outcomes

The service monitored all people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and fully met the expectations of people themselves.

Although the service did not meet national targets for childhood immunisations and screening programmes, the practice demonstrated a proactive approach to improving uptake. Leaders used precise population data to identify specific demographic barriers, such as localised language barriers in individual postcodes. In response, the practice recruited specific language speakers to engage directly with residents to successfully improve vaccination rates in those areas. This extensive and targeted outreach demonstrated that while national benchmarks had not yet been fully achieved, the practice was taking comprehensive, innovative action to address health inequalities and drive improvement. Cervical screening rates were also below the national target, however the practice was actively working to fix this. Staff started targeted outreach in the community, working with local and groups to raise awareness, break down cultural barriers, and increase uptake.
