We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The provider offered patient health checks, raised awareness of health conditions and the importance of health screening.

People living with long-term health conditions were referred or signposted to local services for information, education, and advice linked to their needs. Staff had attended training on support services they could refer patients to. This included support with social issues impacting on their health such as isolation, finance and housing.

Monitoring and improving outcomes

The provider told us how they monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and met the expectations of people themselves.

The provider carried out regular audits and quality improvement work through which information about people’s care and treatment was used to make improvements if required. For example, they told us how they audited prescribing to ensure this was safe and continued to meet people’s needs. They had a system to liaise with and review the needs of patients with health and social care professionals.

The service met the 95% World Health Organisation target for 4 childhood immunisations and the 90% minimum national target for 1 childhood immunisation. The service performed better than the England average in relation to cancer screening programmes for breast and bowel cancer.Based on data from June 2024 the service was below the 80% national target for cervical screening for women in the 25-49 years age group (78%) and the 50-64 years age group (76%). However, more recent unverified data showed that the service had made improvements and had met the 80% target. The service was taking action to maintain uptake of immunisation and health screening.
