Updated 9 July 2025

We assessed East Anglia Ultrasound Services on 29 July 2025.

We carried out this comprehensive assessment to review the progress made against a requirement notice that was served on the provider following our inspection in December 2018. We found that the service had not addressed all the concerns within the requirement notice.

We rated the service as requires improvement. We found 4 breaches of the regulations in relation to safe care and treatment, person centred care, safeguarding and governance. The breach relating to governance was a repeated breach from our last inspection. The service did not always have effective arrangements to monitor the safety and upkeep of its premises and equipment. The service did not always assess or manage the risk of infection. The service did not consistently ensure that staff completed relevant training, demonstrated required competencies, or received formal supervision. Recruitment procedures were not always operating effectively. The service did not provide information in a form accessible to all patient groups. There were no formal audits in place at the time of our assessment. Leaders had failed to ensure the consistent implementation of nationally recognised guidance and established quality and safety standards throughout the service. Leaders did not operate effective governance processes. Systems were not in place to manage performance effectively. Managers did not always identify and escalate relevant risks and issues and identify actions to reduce their impact.

However,

People said staff treated them well and with kindness and did their utmost to accommodate their individual needs. Partner organisations highlighted the collaborative approach taken by staff to support the delivery of high-quality care. Staff felt respected and valued. Leaders were described as visible and supportive.
