The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. The service used recognised assessment tools to identify, assess, and monitor people's needs and risks. For example, the Waterlow assessment tool was used to identify people at risk of developing pressure ulcers. Assessments were reviewed regularly and updated when people's needs changed, helping to ensure care remained effective and responsive to individual risks.

People’s nutritional and hydration needs were assessed and monitored to ensure they received the support required to maintain their health and wellbeing. Where people were identified as being at risk of dehydration, individual fluid intake targets were in place and staff supported them to achieve these. Records demonstrated that fluid intake was monitored, and appropriate action was taken if concerns were identified.

We observed a mealtime and saw people were supported in line with their individual needs and preferences. Staff provided encouragement and assistance where required, helping to create a positive dining experience. A relative told us, “They really do encourage [relative] with meals, and assist [relative], if need be.”

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. People’s relatives spoke positively about the impact the service had on their family members' wellbeing and quality of life. One relative told us, “I have noticed she is much improved since coming in here, the care is so good.”

Where people had specific healthcare needs, the service supported them to access appropriate healthcare professionals. Staff made referrals to specialist services when required and worked collaboratively with external professionals to ensure people received timely and coordinated care. A healthcare professional told us, “The service promotes mutual respect, professionalism, and timely information sharing, enabling concerns to be escalated promptly and care plans to be reviewed collaboratively. This integrated approach supports continuity of care, reduces duplication of services, and ensures patients receive the most appropriate interventions.”
