At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

People’s care and support needs were routinely reviewed to help ensure staff had up to date information to help them support people.

Delivering evidence-based care and treatment

Score: 3

People received evidence-based care and support in line with good practice standards. The provider’s systems helped ensure staff were up to date with national legislation, evidence-based good practice and required standards.

 

The provider ensured people were told about current good practice relevant to their care and were involved in how this was reflected in their care plans. People’s nutrition and hydration needs were met in line with current guidance.

How staff, teams and services work together

The provider worked well across teams and services to support people. There was a collaborative approach to planning and coordinating people's care and support. People and their relatives told us the provider worked with other health and social care services to provide ongoing care and support.

An external health professional told us, “I would like to express my sincere appreciation for the outstanding support provided by the team at JC Michaels – Basildon in caring for [person]. The staff have consistently demonstrated a high level of attentiveness, professionalism, and excellent communication skills, all of which have had a significant and positive impact on [person’s] overall wellbeing.”

 

A social care professional told us about support provided for a challenging and complex care package. They said, “JC Michaels worked with [people] comprehensively to bring a positive outcome for them. They were very professional; they worked really well with all professionals involved with [the people] both internal and external for the wellbeing of these [people].”

 

Staff and management were aware of how to access advice regarding individuals’ health needs, such as from GP's or social workers. Staff communicated with district nurses who visited people to administer certain medicines and shared information where necessary. Staff communicated effectively to keep updated and clear records were kept. This contributed to continuity and consistency of care.

Supporting people to live healthier lives

People were supported to manage their own health, care and wellbeing needs by staff who understood their needs and preferences. People were involved in regularly reviewing their health and wellbeing needs where appropriate and necessary.

 

People were encouraged and supported to make healthier choices to help promote and maintain their health and wellbeing. The management team were able to share some success stories including where a person had been cared for in bed and was assessed as being at end of life. With the care and support provided by care workers the person regained their strength and was able to transfer out of bed independently.

Monitoring and improving outcomes

People were told about their rights around consent and these were respected when delivering person centred care. Staff understood the importance of ensuring people fully understood what they were consenting to and the importance of obtaining consent before they delivered care or support.

 

Staff were skilled in how they obtained people's consent for care adapting their approach to suit individuals’ needs.

 

Where a person was unable to make a specific decision, capacity assessments were completed and best interest decisions were made with the involvement of the person and/or their relatives where appropriate. Examples included administering medicines and delivering personal care.
