We served a Notice of Decision to impose conditions on Monshaw Limited’s registration on 13 June 2025 for failing to ensure safe systems and process were in place to maintain safety, manage risks, and safeguard people, for failing to ensure safe management of medicines, and for failing to implement robust and effective governance systems at Thornhill Nursing Home.

Our current view of the service

Inadequate

Date of assessments were from 2 to 17 April 2025 and this was a planned follow up assessment. The service is a care home providing nursing and personal care for people, some of whom are living with dementia.

The provider was previously in breach of 6 legal regulations in relation to person-centred care, promoting dignity and respect, safe care and treatment, safeguarding, good governance and safe and effective staffing. Improvements were not found at this assessment, and the provider remained in breach of these regulations. ​

There had been a lack of oversight and management of governance systems and processes since the last assessment. Leadership and management had not been consistent, which meant they had failed to drive the required improvements. Risks to people were not managed safely. Medicines were not managed safely. People were not safeguarded from abuse and the staff deployment in the service left people at risk. People did not receive person-centred care and were not always treated with dignity and respect.

However, we did observe some improvements had been made to some areas of the environment, although more work was required. Safe recruitment processes had been followed, and we observed some kind and caring interactions from some staff.

This service remains in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we user our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.

In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.

People's experience of the service

Feedback from people was mixed. Some people told us they felt safe in the service and received enough food and drink. Comments included, “The food is fine, you get enough” and “I feel safe as I know there are people around.”

However, most people told us they did not know who the manager was and felt the home needed more staff. One person told us, “I can hear some people calling out and I don’t know if there is a carer about.” People also reported how there is not much engagement from staff for them. One person told us “All I do is lay here all day.”

Relatives also provided mixed feedback. Themes showed relatives were not always aware of who was in charge or who the manager was, and they felt at times the home needed more care staff. However, l relatives felt the care staff were the best thing about the home and praised staff for being kind and welcoming to them. One relative also told us, “Communication is getting better than last year, the carers have always been good.”
