Governance, management and sustainability
The practice had a governance system that included policies and procedures, which were accessible to staff. There were processes for investigating incidents and accidents, and for receiving and acting on safety alerts.
Improvements should be made to the processes for managing the risks related to carrying out the regulated activities. This included the risks associated with hazardous substances, lone working and the use of sharps.
Governance and oversight of the conscious sedation service must be improved. After the inspection we served a letter of intent requesting further information and assurance that the provision of sedation is being done safely. As a result of this letter, the provider took immediate action to provide us with satisfactory evidence of training in conscious sedation for the required members of staff which was not available on the day of inspection. In addition, the provider arranged for a second medical oxygen cylinder to be in place and for immediate life support training for all staff in the sedation team to be completed before any further sedation was carried out.
Quality assurance and continuous improvement was not embedded within the culture of the practice. An x-ray audit had been completed, however, audits of infection prevention and control, dental care records, antibiotic prescribing and conscious sedation had not.We discussed this with staff and were assured it would be addressed and rectified.
Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records and complied with General Data Protection Regulations.
Concerns and complaints were responded to appropriately.
Staff feedback was obtained through meetings and informal discussions.
The practice gathered feedback from patients, the public and external partners, and responded accordingly.
