Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

Staff told us they were updated on changes to people’s needs via a group messaging service.

People and their relatives told us they had been involved in discussions about their or their loved one’s care needs when they first started receiving support. People also told us they were kept up to date with changes and were consulted when amendments were needed to their care and support.

The documentation we reviewed showed that people’s needs were consistently monitored and reviewed, which ensured staff had up‑to‑date information about the care each person required.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

People had specific care plans in place for healthcare needs, such as diabetes and epilepsy. These supported staff to provide the correct care to people who used the service.

People who used the service told us that care staff were knowledgeable and kept them informed of any changes.

Staff told us they had access to up‑to‑date care plans and information that enabled them to provide safe and consistent support. One staff member told us, “We complete mandatory training annually, including safeguarding, mental capacity, and fire safety. We also receive specialist training based on resident’s needs.”

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

When issues arose that required input from other health professionals, staff supported people to make contact and worked closely with relatives to ensure the person received the support they needed. One staff member told us, “When I raised a concern about a resident’s mood changes, the manager arranged a GP review and additional wellbeing checks that same day.” People who used the service corroborated this, and told us, “They do notice if there is something wrong and sometimes, they have rang a doctor for them.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The feedback we received from people and their relatives was consistently positive. Relatives told us staff supported their family members to achieve good outcomes, which included ensuring they had access to appropriate health professionals when needed. This helped people receive timely advice and contributed to their overall well-being.
