During an assessment of Diagnostic imaging

Date of assessment: 16 April – 12 May 2025.

We reviewed this service due to concerns raised and the location not having had a recent inspection. During the inspection, we spoke with staff, leaders and people who used the service and found staff were caring towards people. However, there was a lack of communication from leadership to the staff. We found some senior managers were not visible and staff did not feel comfortable to raise concerns to senior managers, although there were processes in place to raise concerns. We looked at people’s records, policies and other documents relating to the service and found the service was providing suitable care for people. We found the service was not supporting staff adequately and there was a disconnect, however, this was not impacting the service provided to people. The service was in breach of legal regulation in relation to good governance, particularly shared direction and culture.
