Responsive – this means we looked for evidence that the provider met people’s needs.

At our last inspection this key question was rated required improvement. Following this inspection this has now changed to good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care

Score: 2

Despite people being at the centre of their care and support, some improvement was required to ensure person-centred information was clearly documented about people. Some information around people’s history and backgrounds was not completed or lacked information. For example, for 1 person who was diagnosed with a complex mental health condition, there was minimum information around how this impacted them. This might mean some agency staff would not have enough information to support this person. We highlighted this to the registered manager who has since updated and reviewed all care plans.

Care provision, Integration and continuity

Score: 3

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. People were well supported by staff from different cultures and backgrounds. People’s dependency tools which recorded levels of care needs and interventions reflected the staffing levels in the home. Additionally, if people required 1-1 support or support to attend an appointment, this was done in consultation with their relatives.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. Information was available in easy read, large print, and verbal formats to support people with additional needs.

Listening to and involving people

Score: 3

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. Everyone we spoke with and the staff confirmed the registered manager was always ‘hands on’ and listened to any concerns they shared.

Equity in access

Score: 3

The provider made sure that people could access the care, support and treatment they needed when they needed it. People were supported promptly and referred to agencies such as district nurses, Speech and Language Teams (SALT) and psychiatrists in a timely manner.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Care notes included dates and times when staff followed up on appointments for people along with any follow up actions or recommendations.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. Where appropriate and in accordance with people’s individual needs they had an end of life pathway in their care plan, which had been completed in consultation with them, if they were able, or under a best interest process with their families.
