Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment we looked at 2 quality statements and the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider did not always make sure people’s care and treatment were effective because they did not always check and discuss people’s health, care, wellbeing, and communication needs with them.

 

Reviews of people’s care were undertaken when they moved into their home and people were involved with these if they were able to, however these had not always been reviewed. The management team arranged review dates at the time of inspection with people and their advocates where appropriate. The registered manager had also arranged training sessions in relation to person centred reviews with the managers in the houses to enhance their knowledge.

 

People were not in every house been supported to have meetings to discuss their needs. Discussions were held in most houses, however, were not always formally recorded. We saw evidence of these captured in relation to informal discussions. We discussed this with the management team who have immediately discussed ways to capture this which is appropriate for each person.

 

Support plans were formulated using a variety of methods, such as meeting with people's social workers and obtaining health history. People's likes, dislikes and choices were documented throughout their support plan in line with their assessed needs.One staff member told us, “I am made aware of people's assessed needs through their care plan, updates are communicated to me through our communication book, this is where all staff read and pass information of any latest developments.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Staff recognised individual’s communication needs, and all people had communication care plans to ensure people knew about their rights and how people may refuse or give consent to specific decisions. We did discuss with the management team in relation to Mental Capacity Assessments which needed reviewing. They had already recognised this and had requested support from social workers.

 

People and their relatives told us they were always asked for consent before any care was delivered. One person said, “I have lots of friends and go to the cinema and dancing as well as swimming.” We observed staff had completed an easy read document making people aware we were visiting their home.” Another person said, “I get to choose where I go out and where I go on holiday, guess what that’s where im going as well where I have chosen.” We spoke to a staff member who told us, “We encourage choice all the time from clothes to what food people would like, we give options and choices all the time.” A staff member told us, “By involving them in decision making and providing the information they can understand about their right to be involved so they won’t be left out.”
