The service had a very clear shared vision, strategy and culture. This was based on transparency, equity, equality, human rights, diversity, inclusion and engagement. Staff and leaders demonstrated an exceptional understanding of the challenges and the needs of people and their communities.
All staff had contributed to the development of the service’s vision and strategy, which was kept under review. Leaders monitored and reviewed progress against the delivery of their strategy. The practice demonstrates a highly inclusive, values-driven, and aspirational culture, where quality improvement, equity, and patient-centred care are central to the shared vision.
Leaders are visible, approachable, and proactive, and foster a culture of openness, trust, and psychological safety. Staff report feeling confident to raise concerns, share ideas, and challenge practice, with evidence that feedback is actively listened to and acted upon. This has created an environment of continuous learning and innovation, rather than blame.
There is a strong emphasis on staff wellbeing, development, and empowerment, with regular engagement through meetings, supervision, and informal support. Staff are encouraged and supported to develop their roles, contribute to service improvement, and take ownership of key areas, including work to reduce health inequalities. The culture is reflected in high levels of teamwork, morale, and staff engagement, with a shared commitment to delivering the best outcomes for patients. This is particularly evident in areas such as learning disability care, where staff demonstrate pride, ownership, and a proactive approach to improving services.

Governance, management and sustainability
The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support.
All staff we spoke with were clear on their individual roles and responsibilities. Leaders proactively supported staff and regularly met with them to complete appraisals and performance reviews. Leaders clearly recorded any actions that arose from these meetings and shared these with staff. Governance arrangements are clear, structured, and effective, providing oversight of risk, quality, and performance. Systems are in place to manage audits, significant events, and complaints, with evidence of learning and improvement.
The practice demonstrates good awareness of current and future challenges, supported by forward planning and workforce development, ensuring sustainability and resilience. Staff discussed emerging risks and potential improvements to the service. Staff took confidentiality and information security seriously, which included verifying people’s identity during remote consultations. Staff submitted data and notifications to external agencies as required.
