Condition of this registration relating to carrying out this regulated activity
The registered provider must not provide the regulated activity of personal care to any new service users under the location registration Lifeways Community Care Limited (Leicester) without the prior written agreement of the Care Quality Commission.
The registered provider must ensure all staff are appropriately trained, assessed as competent, and supported to undertake their roles safely and effectively.
a. By 13 August 2026 date, all staff who administer Buccal Midazolam as a delegated healthcare task must complete training and competency assessments in line with the delegated clinician responsible and any associated clinical guidance. Competency assessments must be completed and signed off by an appropriately trained and competent assessor before staff undertake this task unsupervised.
b. By 20 August 2026 date, all staff responsible for the administration of medicines must successfully complete accredited medicines management training and demonstrate competence in the safe administration, recording, storage, handling, and management of medicines.
c. By 20 August 2026 date, all staff responsible for supporting people with moving and positioning must receive face-to-face practical training in moving and handling, including the safe use of all equipment utilised within the service. Staff must not undertake moving and handling tasks until they have been assessed as competent.
The registered provider must ensure that a suitably skilled and experienced person is appointed with responsibility for the oversight of each staff team delivering the regulated activity. The person must undertake and document oversight activity with each staff team at least once every seven days to monitor staff practice and identify risks relating to the quality and safety of care, including risks associated with the development of a closed culture. This oversight must include review of incidents, accidents, safeguarding concerns, complaints, whistleblowing concerns, staff conduct and care practices. Actions identified through such oversight must be documented and monitored to completion.
a. The registered provider must ensure an appropriately trained, independent, and competent person undertakes weekly medicines governance reviews. These reviews must include, but not be limited to:
(i) Accuracy and completeness of MAR charts.
(ii) Prescribing arrangements and liaison with prescribers.
(iii) Consent and mental capacity assessments.
(iv) Best interest decision-making processes where applicable.
(v) PRN and other medicines protocols.
(vi) Authorisations and delegated healthcare tasks.
(vii) Staff practice, competency, and compliance with medicines policies.
b. The registered provider must ensure that an appropriately trained and competent person undertakes comprehensive reviews of all moving and handling arrangements for people requiring support on a monthly basis. Reviews must include:
(i) Current moving and handling assessments
(ii) Equipment suitability and maintenance.
(iii) Care planning and risk assessments.
(iv) Referrals to relevant health and social care professionals.
(v) Monitoring of staff practice and compliance.
(vi) Ongoing review to ensure risks are identified, mitigated, and managed safely.
