Effective
We looked for evidence that people and communities had the best possible outcomes because their needs were assessed. We checked that people’s care, support and treatment reflected these needs and any protected equality characteristics, ensuring people were at the centre of their care. We also looked for evidence that leaders instilled a culture of improvement, where understanding current outcomes and exploring best practice was part of their everyday work.
At our last assessment we did not rate this key question. At this assessment the key question has not been rated as we did not review the whole of the key question.
Managers produced policies and standard operating procedures for staff guidance on how to carry out their roles. These were based on national guidance and reviewed annually. However, some detail was missing from some of the policies had not been amended to show current practice.
This service scored 12 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Delivering evidence-based care and treatment
The service planned and delivered people's care and treatment with them, including what was important and mattered to them. Staff did this in line with legislation and current evidence-based good practice and standards.
Staff were trained for their roles and updated their skills as required. The service was accredited by nationally recognised first aid, health and safety training organisations such as Qualsafe and QNUK (Qualifications Network) and were able to provide staff with training relevant to their roles.
Staff followed policies and were informed if there were any changes to actions needed. Staff knew where to access policies if they needed to read them. Staff described how they followed policies in their daily work. This included following safeguarding procedures to ensure patients were left in their homes safely and how to report any concerns identified.
Senior managers reviewed policies annually and included national guidelines. However, we found some policies lacked detail or were not amended to ensure they were up to date. For example, 2 policies referred to the Healthcare Commission which is no longer active.
