Effective
Provider used the pre-admission assessments and risk assessment tools to develop people’s care plans. Risks to people were regularly reviewed so they continued to reflect people's current needs and expectations. People were encouraged and supported to make their own decisions where possible and their consent to care and treatment was sought. People’s consent was sought before any care or support was given.

Assessing needs
People’s needs were assessed when they were admitted to the home and reviewed regularly or as their needs changed. A relative told us, “They obtained a detailed background history and relevant information in order to optimise individualised quality care, and this is reflected in care plan information and care administration.” Another relative said, “We did an assessment initially. [Name] fully understands their individual needs.”
Staff knew how to meet people’s needs as their needs had been assessed and staff had clear guidance on how to meet people’s needs.
The provider had systems to assess people’s needs and provide plans of care to mitigate known risks and provide care that met people’s needs.

Consent to care and treatment
People were asked for their consent before staff provided their care. People told us staff respected their decisions and choices made on a daily basis. Where people were unable to make decisions about their care, staff followed the correct procedures to ensure safeguards were in place. A relative said, “We had lots of initial meetings and included a capacity assessment.”
Staff understood their role in ensuring people provided their consent to the care they provided.
There were systems in place to assess people’s capacity to give their consent to care and have plans of care to manage this. Where people lacked capacity this was documented clearly with best interest decisions made about people's care. Any conditions to deprive people of their liberty had been adhered to.
