Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, well-being and communication needs with them.

The registered manager carried out assessments of people’s health, care, well-being and communication needs before care commenced. People and, where appropriate, their relatives were involved in these assessments to ensure care reflected their wishes and preferences, routines and desired outcomes.

Care plans were personalised and contained clear information about how people wished to receive support and any risks associated with their care. Needs were reviewed regularly and updated when people’s circumstances changed, helping to ensure care remained effective and responsive. Staff understood people’s individual needs and used assessment information to provide person-centred support that promoted people’s well-being and independence. One staff member said, “It’s tailored care for one person at a time. All their needs and individual care are met on each call. We discuss each person’s preferences and record these after each visit.”

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

Staff received training in best practice to ensure they supported people in line with national standards and guidance. The registered manager worked closely with other professionals and told us they would seek advice and support if necessary. We saw evidence in care plans to show professional guidance was followed by carers. Care records contained detailed information about people’s mobility needs and any associated risks, including the risk of falls.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Care records reflected shared decisions and multidisciplinary input, ensuring care plans remained accurate and responsive.

Systems were in place to ensure staff communicated effectively with each other, families and managers. One relative said, “The care staff complete great notes, which I appreciate. They [staff] are very responsive, any questions I ask they get back to me immediately even on a weekend.”

The registered manager said, “It is very important that we have dialogue with not just the client, but whoever is down as their next of kin, so that they feel they can talk to us if they feel that something’s not quite right or if they feel they want to tweak something or change something. They can approach us in whatever way suits, face to face, email or phone call.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Information about people’s health needs was documented in care records. These were regularly reviewed, which helped staff to monitor health concerns, identify changes, and respond appropriately. Staff told us they would bring any concerns about a person’s health to the registered manager for advice or action if needed. The provider collaborated with other professionals to support people’s health and wellbeing.

For one person staff helped coordinate hospital appointments, helped bring overdue healthcare reviews back on track and arranged a comprehensive medication review.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The planning and delivery of care focused on what was important to people, such as maintaining their independence, improving well-being and achieving personal goals. People were involved in reviewing their care and staff understood how to deliver personalised support which reflected people’s preferences and wishes.

The registered manager maintained oversight through quality assurance systems, audits and feedback. Where concerns or emerging needs were identified, action was taken to improve outcomes and ensure care remained effective. As a result, outcomes were positive, consistent and met both clinical expectations and the expectations of people.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

People told us they were able to consent to their care. One person said, “They [staff] always ask permission.” People and their relatives where appropriate, had been involved in their pre-care assessments which helped ensure care provided was in line with their wishes. They had signed consent forms to agree to care being provided in line with their assessed needs.
