Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to support them when their needs changed. People and their relatives told us they were involved in planning care which covered the support they needed including their physical, emotional, and social needs. Their care options were fully discussed with them, as part of the assessment process, and it was explained what they could and could not expect from the service. They were informed of any required adjustments to their care plans, and their consent was sought. A person told us, “I get to do the things I like. I like music and they [staff] put it on when I want it.”

Care provision, Integration and continuity

Score: 3

We did not look at Care provision, Integration and continuity during this assessment. The score for this quality statement is based on the previous rating for Responsive.

Providing Information

Score: 3

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. Since 2016 onwards all organisations that provide publicly funded adult social care are legally required to follow the Accessible Information Standard (AIS). The standard was introduced to make sure people are given information in a way they can understand. The standard applies to all people with a disability, impairment or sensory loss and in some circumstances to their carers. People, where appropriate, and their families were informed how to access their health and care records and decide which personal information could be shared with care staff and healthcare professionals when people were supported by the service. People received information in a timely way that met best practice standards, legal requirements and was tailored to individual needs. People and their relatives were provided with clear, and transparent information regarding contracts and charges, that followed consumer rights best practice. Information about people that was collected and shared met data protection legislation requirements. One relative told us, “There is always a clear 2 way conversation so everyone has the most up to date information.”

Equity in experiences and outcomes

Score: 3

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. There were systems and processes that protected people regarding their care, treatment, and support. This was by promoting equality, removing barriers, protecting their rights, and making them feel their experiences of discrimination, and inequality were listened to and acted upon to improve their and other people’s care. This meant people experienced equity in experience, and access.
