We did not assess all quality statements in this key question and therefore did not re-rate well led at this time. The previous rating of inadequate still stands.

We looked for evidence the service had improved the governance around maintaining staff records, giving staff the opportunity to feedback to the service and sharing learning from incidents with staff. We found that some improvements had been made but the breach of regulation 17 remained.
