Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. This is the first assessment for the patient transport and diagnostics services. This key question has been rated requires improvement. We were unable to rate the diagnostics service.

This meant people were not safe and were at risk of avoidable harm.

The service was in breach of legal regulation in relation to people’s safe care and treatment, safeguarding and governance. Governance was not managed at the service, and this left people who used the service at risk. The service had a negative learning culture; employment and other arrangements made it difficult for both staff and patients to raise concerns. Staff did not always manage risks to patients and themselves. Not all staff understood how to protect patients from abuse. Staff employment arrangements did not provide assurance for ongoing safe and effective service provision.
